Community and Social Service Specialists
CANDIDATE RESPONSIBILITIES:
• Draws conclusions from reading/reviewing medical records, and then writes corresponding case- notes which highlight the key-points.
• Utilizes established guidelines and internal protocol to determine if client meets medical eligibility criteria based on review of medical records and client psychosocial history.
• Applies knowledge of aging process, activities for daily living and medical conditions to written case-notes.
• Justifies medical eligibility recommendation through written narratives which summarize client’s current situation, limitations and needs.
• Presents case-notes and medical eligibility recommendations to Registered Nurse for approval, and adjusts to professional feedback provided by Registered Nurse and/or Supervisor
EDUCATION REQUIREMENTS:
The candidates must meet the minimum qualification of:
• A bachelor’s degree or higher from an accredited college or university with a major in a related field,
OR
• Professional experience totaling four (4) or more years in a related field
OR
• A combination of education and professional experience totaling four (4) or more years in a related field.
